Can anyone tell me the reasoning behind  the "Nipple bolt" part of the compression arm is for.

I've been staring at my replacements for ages wondering what on earth they actually do. Am I supposed to milk it? It is for extra sustenance from the Z gods? Answers on a postcard please.
