I have two part rotors - wanting to replace the discs

Hi,

I'm wanting to replace the discs with new ones. They are two part rotors - does anyone know what make they are? (I am led to believe they are DBA) and where I can get replacement discs? (or do you need to buy the whole kit?)

 

This is the best picture I have right now, let me know if you need something better.
